Keto and PCOS — did it actually help your symptoms?

I was diagnosed with PCOS last year. My endocrinologist mentioned keto as something that can help with PCOS but wasn't specific about why or how much. Has anyone with PCOS done keto and what was the actual impact on your symptoms?

PCOS and keto have a strong documented relationship because the root cause of most PCOS is insulin resistance. When insulin is chronically elevated, the ovaries produce excess androgens (testosterone-like hormones) which cause the signature PCOS symptoms: irregular periods, acne, excess hair growth, weight gain particularly around the midsection, difficulty conceiving.

Keto addresses insulin resistance directly by minimizing the carbohydrates that spike insulin. Several studies show meaningful improvements in:

— Menstrual regularity (many women see periods normalize within 3-6 months)
— Androgen levels (free testosterone often drops significantly)
— LH/FSH ratio normalization
— Reduced acne and excess hair growth
— Improved fertility in some cases

This is one of the conditions where keto has the strongest evidence base, not just anecdote.

PCOS for 8 years, diagnosed in my late 20s. Three months of keto and my period came back for the first time in 14 months without medication. That alone would have been enough. One practical note: PCOS keto may require stricter carb limits than standard keto (some endocrinologists recommend under 15g net carbs rather than 20g for better insulin response).
