Keto with hypothyroidism — anyone done this long term?

I have Hashimoto's hypothyroidism (diagnosed 4 years ago, on levothyroxine 75mcg). My doctor said keto should be "fine in theory" but she also admitted she doesn't know much about it specifically with thyroid conditions.

I've been reading conflicting things — some say very low carb can suppress thyroid function further, others say it has no effect or even helps. Has anyone here done keto long-term with hypothyroidism? What was your experience?

This is a real nuance in the keto world. The research is genuinely mixed so I'll share what seems to happen in practice.

Some people with hypothyroidism do great on keto — the weight loss, reduced inflammation, and blood sugar stabilization can actually improve thyroid-related symptoms. Others find that very strict keto (under 20g) seems to worsen thyroid numbers, particularly T3 levels.

The current thinking is that some carbohydrate is needed for optimal T4 to T3 conversion. This doesn't mean you can't do keto — it means you might do better at 30-50g net carbs (cyclical or targeted keto) rather than strict 20g, especially if you feel consistently cold or more fatigued than expected.

Practical advice: get your thyroid labs done 3 months into keto and compare to baseline. TSH, T3, T4, and reverse T3. If numbers have shifted, adjust your approach.

Hashimoto's specifically has an autoimmune component. Keto's anti-inflammatory effects can actually benefit Hashimoto's patients — several small studies show reduced antibody levels (TPO and TgAb) on low-carb diets.

The T3 suppression concern is real but often overstated. If you eat adequate protein (which keto requires) and don't crash your calories, T3 suppression is usually minimal. The issue mainly appears in people doing keto AND severely restricting calories simultaneously.

Monitor your labs, talk to a doctor who's familiar with both keto and thyroid conditions if possible, and don't panic at short-term fluctuations in the first few months while adapting.

I have subclinical hypothyroidism (not on medication, just monitoring). Did keto for 14 months. My TSH actually improved over that time — went from 3.8 to 2.1. Can't say definitively keto caused it but it certainly didn't make it worse.

I do eat 30-35g net carbs, not strict 20g, which might be relevant. I also eat plenty of selenium-rich foods (brazil nuts, sardines) which support thyroid function.

Same boat — Hashimoto's on levothyroxine. What helped me specifically: taking my thyroid medication 4+ hours away from both my keto coffee (the fats may interfere with absorption) and my electrolyte supplements. Timing matters more than most people realize for thyroid meds.
