I've been seeing MCT oil recommended constantly in the keto world. I bought a bottle but haven't started using it because I read that it can cause severe digestive problems and I'm scared to try it before work. What's the real deal with MCT oil? Does it actually do anything meaningful for ketosis or is it mostly marketing? And what's the right way to start if I do use it?
MCT oil on keto — benefits, dosing, and which brand

MCT oil is one of the few keto supplements that has genuine scientific backing — it's not just marketing. Here's what it actually does:
MCTs (medium-chain triglycerides) bypass normal fat digestion and go directly to the liver where they're rapidly converted to ketones. This means you can raise blood ketone levels faster and maintain them more easily, especially during the adaptation phase.
Real benefits: faster ketosis entry, sustained mental energy (the "brain fog lifts" effect people report), appetite suppression (ketones are appetite-suppressing by nature).
The digestive issue is real but manageable. Start with 1 teaspoon per day in coffee or food. Increase by 1 tsp every 3-4 days. Most people can reach 1-2 tablespoons per day without issues. The problem happens when people pour 2 tablespoons in their first coffee — that will absolutely cause GI distress.
My starting protocol: 1 tsp in my morning coffee for week 1. Then 2 tsp for week 2. Then 1 tbsp by week 3. Zero digestive issues following this ramp. Made the mistake of starting with a tablespoon on day one my first time and spent half the morning in the bathroom. Don't do that.
On brands: Bulletproof Brain Octane is pure C8 (the most ketogenic MCT) and works best but expensive. Sports Research MCT oil is a good C8/C10 blend at a much better price. Avoid anything labeled "MCT" that's mostly C12 (lauric acid) — it behaves more like regular fat.
MCT powder is worth considering if the digestive issue concerns you — it's gentler because the powder has a slower release. Perfect Keto makes a good one. You can put it in coffee or smoothies. Not quite as potent as oil but much more forgiving for beginners and easier to travel with.
Honest assessment after 3 years using it: it's a nice addition but not transformative if you're already fully fat adapted. The biggest benefit is in the first 4-6 weeks of keto when you need all the help you can get staying in ketosis. After that the marginal benefit decreases as your body gets efficient at burning fat naturally.
Starting with 1 tsp tomorrow morning then. Will wait until the weekend to experiment so I'm not at the mercy of my commute if anything goes wrong. The C8 vs C12 distinction is really useful — I was about to buy whatever was cheapest on Amazon which would probably have been mostly lauric acid.
Adding one practical tip: take it with food, not on an empty stomach, for the first 2-3 weeks. Even a handful of nuts before your MCT coffee can significantly reduce the digestive response while your gut adapts.
