Had a birthday dinner and ate a full piece of cake plus pasta. I was 4 weeks in and doing so well. How do I get back into ketosis as fast as possible? I feel awful physically and mentally.

Recovery protocol — fastest way back:
Day of/morning after: Don't try to "make up for it" by eating nothing — that adds stress to recovery. Eat normal keto meals (eggs and bacon breakfast, fatty meat for dinner).
Fastest method: 24-36 hour fast starting the morning after. Drink water, black coffee, electrolytes. Fasting depletes glycogen stores faster than eating even strict keto. Most fat-adapted people can be back in ketosis in 24-36 hours versus 48-72 hours of just eating keto food.
Add exercise: A 30-45 minute walk the morning after burns glycogen directly. Light cardio, not intense.
Extra hydration: 3-4 liters of water with electrolytes. The carb refeed causes water retention — flushing it speeds the physical recovery.
Most fat-adapted people are back in ketosis in 1-3 days. Non-adapted people may take 3-5 days. You're at 4 weeks, so the adapted timeline applies.
The physical feeling awful is partly mental and partly real blood sugar fluctuation after weeks of stable ketosis. The mental part: forgive yourself immediately. One meal is one meal — it doesn't define your commitment or erase your progress. The physical part passes within 24 hours as your blood sugar restabilizes.
Also: don't weigh yourself for 3 days after a carb refeed. The water weight from stored glycogen will look like 3-5 lbs of weight gain. It's not fat. It will drop within 3-4 days. Seeing that number on the scale before it drops will psychologically mess you up unnecessarily.
